How we protect your child's health information

This notice explains how School of Imagination, Inc., including Happy Talkers, uses and shares your child's health information, and the rights your family has.

Who this covers

All of our programs and team members

This notice applies to every School of Imagination program and every staff member, clinician, and volunteer who works with your child's health information. Because our patients are children, a parent or legal guardian can use the rights in this notice on their child's behalf. We follow federal privacy law (HIPAA) and California privacy law.

Your rights

You have the right to:

To use any of these rights, contact our Privacy Officer.

See and get a copy of your child's health records, on paper or electronically. We may charge a reasonable, cost-based fee.
Ask us to correct information you believe is wrong or incomplete. If we say no, we will explain why in writing.
Choose how we contact you, such as a different phone number, address, or email.
Ask us to limit what we use or share. If you pay for a service in full yourself, you can ask us not to share it with your health plan.
Get a list of certain times we have shared your child's information.
Get a copy of this notice at any time, on paper or electronically.
Have someone act for you. A parent, legal guardian, or other legal representative can use these rights for your child.
File a complaint if you feel your rights have been violated. We will never retaliate against you for filing one.
How we use information

We use and share information only as needed

To provide care. Our team shares information to coordinate your child's services, and we may share with other providers treating your child.
To run our programs. We use information to improve care, train staff, and manage our services.
To bill for services. We share information with your health plan, Regional Center, or other funding source.
With trusted partners who help us, such as billing and records systems. They must protect your information too.

We may also share information when the law allows or requires it, such as to report suspected child abuse or neglect, prevent a serious threat to health or safety, support public health, respond to health oversight agencies, comply with a court order or other legal process, or meet other legal requirements.

Fundraising. As a nonprofit, School of Imagination may contact you about supporting our programs. You can tell us at any time that you do not want to receive fundraising messages, and your choice will never affect your child's care.

Your written permission comes first
  • We ask for your written permission before any use not described in this notice. You can change your mind at any time, in writing.
  • We never sell your child's information or use it for marketing without your written permission.
  • Tell us who in your family or circle of care may receive updates, and we will follow your wishes.
Our responsibilities

What we promise your family

We are required by law to protect your child's health information, follow the practices in this notice, and let you know promptly if a breach affects your child's information. We may update this notice, and changes will apply to all information we hold. The current version is always on this page and available from our front office.
